Type of service provided
The service is registered with the Care Commission and provides Housing Support and Care at Home services to individuals over the age of 18 years who have experienced or been affected by homelessness and associated issues. The service can support a maximum of 8 adults at any one time and operates a comprehensive multi-agency risk assessment order to maximise the opportunity for the service users to maintain their accommodation. The overall aim of the service is to provide appropriate and flexible support which meets the individuals needs of each service user. The available direct support hours on a weekly basis are 15 – 17 hours.

How to make a referral and who to contact
Referrals for the service will come via the allocated Assessment Co-ordinator, Homeless Persons Team or directly from the Commissioning officer. A completed Loretto referral form (including first stage risk assessment) and a completed CC1 and/or Integrated Assessment are required to progress referrals.
