Type of service provided
The services provide support to individuals experiencing enduring mental health problems.
An individual packed of care and support is put in place to support the Service User to be as independent as possible within their home and the wider community.
The service is provided over a 24 hour period through a variety of shifts from 8am to 11pm, with a sleepover facility between 11pm and 8am. The Blantyre service also provides an outreach service over 7 days from 9am to 8pm.
Individual care and support packages might include:
• Support with medication
• Budgeting
• Daily living skills
• Personal care
Service users are encouraged and supported to identify and to meet their own goals and aspirations.
The South Lanarkshire Services staff team consists of:
1 Projects Co-ordinator
3 Team Leaders
12 Support Workers
14 Support Assistants
A Multi Disciplinary approach is taken, with support staff working closely with Community Psychiatric Nurses, Consultant Psychiatrist, Social Worker, Occupational Therapists, General Practitioner, Advocacy and other relevant agencies.
The service is registered with the Care Commission as a combined Housing Support and Care at Home service and regular inspections take place.
The Organisation also carries out a yearly Evaluation of the Service, and internal audits are carried out on a regular basis by Care Management.
