How to make a referral and who to contact
The primary reason for referral to the service will be directly related to the individual being affected by homelessness and having alcohol related issues. The current referral process requires completion of a Loretto referral form and a Loretto Health and Safety checklist along with a suitable detailed assessment from the referring Agency/Care Manager. The completed referral paperwork should then be forwarded to the Senior Commissioning Officer :-
